Ascent serves patients in need of home health care services in the Roland, Oklahoma area, including Roland, Moffett, Muldrow, Sallisaw, Dora, Liberty, Nicut, Long, Maple, Poteau, Spiro, Pocola, Poteau, Heavener and surrounding areas.

We are dedicated to providing superior care to local families and individuals who have recently been hospitalized for surgery or illness, and wish to recover in their home, assisted living facility or senior housing.

  • My doctor has prescribed home health care
  • I need either skilled nursing care on an intermittent basis or therapy services (i.e., physical/occupational/speech therapy)
  • I am restricted in my ability to leave home (homebound). This means your condition should be such that there exists a “normal inability to leave home” and doing so would require considerable and taxing effort.

Ascent is an Oklahoma licensed and Medicare certified home health care agency through the Community Health Accreditation Program (CHAP), the first independent, non-profit accrediting body for community based health care organizations in the United States.  We have chosen to go through this demanding process as a testament to our commitment to a defined set of internationally accepted standards, to ensure that our services remain among the best in home health care.

All of our employees undergo an extensive pre-employment screening which includes a criminal background check, employment references, drug screen, abuse and sexual offender registry, and felony and probation checks. Each one of our employees is fully bonded and insured in the state of Oklahoma.

Taking a Team Approach to Patient-Centered Recovery

We know that the need for care varies from client to client; therefore we customize each plan of care to fit the situation.  Our interdisciplinary team works with your physician to create a plan, then monitors, assesses, and revises the plan as necessary. In addition, we will provide physician updates and patient outcome reports as well as measuring your satisfaction through regular communication and patient satisfaction surveys.
